What Is a Syndesmosis Injury?

A syndesmosis injury, often called a “high ankle sprain,” involves damage to the ligaments that connect the tibia and fibula bones above the ankle joint. These injuries are

common in field and court sports

where twisting or external rotation forces occur — especially during tackles or awkward landings.

Common symptoms include:

  • Pain above the ankle (not just around the joint)

  • Difficulty weight-bearing, especially when walking uphill or turning

  • Swelling and bruising higher up the leg

Surgical vs Non-Surgical Syndesmosis Management

Treatment depends on the severity of the injury:

Grade I–II (Stable Injuries)

These can usually be treated conservatively with progressive loading and rehab. We focus on restoring function, strength, and sport-specific capacity early while avoiding aggravation.


Grade III (Unstable or Complete Tear)

These often require surgery — commonly with:

1) Tightrope fixation (suture-button stabilisation)

2) Screw fixation between tibia and fibula
