What Is an MCL Injury?

The medial collateral ligament (MCL) runs along the inside of your knee and plays a vital role in stabilising the joint — particularly during cutting, lateral movements, and contact.

MCL injuries commonly occur due to valgus stress, which forces the knee inward — often during tackles, sudden pivots, or when landing awkwardly in sport. Field sports like football, rugby, netball and basketball are particularly high-risk for this mechanism.

Common Symptoms of an MCL Injury:

  • - Pain and swelling along the inner (medial) side of the knee

  • - Instability or looseness when changing direction or pivoting

  • Discomfort during walking, jogging, or descending stairs

  • Difficulty with sport-specific tasks like cutting, jumping, or contact drills

  • In more severe cases, difficulty fully extending or flexing the knee

Why Early Rehab Matters

A proper rehab plan from the start can significantly speed up healing and reduce the chance of longer-term issues. At Pivot Bundoora, we focus on progressive loading, movement correction, and return-to-play testing from day one.

Early rehab helps:

Promote optimal ligament healing with controlled movement and loading

Prevent the development of compensatory movement patterns that can overload other joints (e.g. hip, ankle)

Accelerate return to sport by addressing the demands of cutting, agility, and contact-specific drills

MCL Injury Rehab FAQs

Do I need a knee brace after an MCL injury?

For Grade II or III injuries, a hinged brace is often recommended during the early phase of rehab to limit excessive valgus movement.

As strength, control, and pain improve, we progressively reduce your reliance on bracing and support you in returning to brace-free function.

FAQ image

Can I return to sport without surgery?

Yes — the vast majority of MCL injuries are successfully managed without surgery.

Conservative rehab is typically effective, even for moderate-to-severe strains.

Surgery is only considered if there's multi-ligament involvement (e.g. ACL + MCL) or if the ligament fails to heal after extensive rehab.

FAQ image

How do you know when I'm ready to return to sport?

We use objective testing — not just “time off” — to guide return to sport. This includes:

  • Strength testing (quad, hamstring, adductor symmetry)

  • Hop and landing control tests Change-of-direction drills under pressure

  • Sport-specific reactivity and fatigue testing

Only once you pass these benchmarks — and feel confident — will we clear you to return.
