What Is an MCL Injury?
The medial collateral ligament (MCL) runs along the inside of your knee and plays a vital role in stabilising the joint — particularly during cutting, lateral movements, and contact.
MCL injuries commonly occur due to valgus stress, which forces the knee inward — often during tackles, sudden pivots, or when landing awkwardly in sport. Field sports like football, rugby, netball and basketball are particularly high-risk for this mechanism.
- Pain and swelling along the inner (medial) side of the knee
- Instability or looseness when changing direction or pivoting
Discomfort during walking, jogging, or descending stairs
Difficulty with sport-specific tasks like cutting, jumping, or contact drills
In more severe cases, difficulty fully extending or flexing the knee

ATHLETES RETURNED TO SPORT
ATHLETE REHAB HOURS COMPLETED
A proper rehab plan from the start can significantly speed up healing and reduce the chance of longer-term issues. At Pivot Bundoora, we focus on progressive loading, movement correction, and return-to-play testing from day one.
Early rehab helps:
Promote optimal ligament healing with controlled movement and loading
Prevent the development of compensatory movement patterns that can overload other joints (e.g. hip, ankle)
Accelerate return to sport by addressing the demands of cutting, agility, and contact-specific drills
High Performance Training Throughout Your Entire Season.
Phase 1 – Acute Management
Swelling and pain control using manual therapy, compression, and gentle mobility
Restoration of range of motion (ROM)
Bracing or taping support where indicated (especially for Grade II–III injuries)
Phase 2 – Strength Development
Targeted strength for quads, hamstrings, adductors, and calves
Isometric, concentric, and eccentric loading to rebuild joint control
Introduction of balance and proprioception drills to support stability
Phase 3 – Movement and Running Integration
Return to linear running under physiotherapist guidance
Agility and change-of-direction drills
Contact preparation: cutting under pressure, lateral resistance, and reactive drills
Phase 4 – Testing and Return to Play
Video feedback for movement analysis
Hop testing and strength benchmarks to confirm readiness
Sport-specific reloading based on your code (football, netball, rugby, etc.)

For Grade II or III injuries, a hinged brace is often recommended during the early phase of rehab to limit excessive valgus movement.
As strength, control, and pain improve, we progressively reduce your reliance on bracing and support you in returning to brace-free function.
Yes — the vast majority of MCL injuries are successfully managed without surgery.
Conservative rehab is typically effective, even for moderate-to-severe strains.
Surgery is only considered if there's multi-ligament involvement (e.g. ACL + MCL) or if the ligament fails to heal after extensive rehab.
We use objective testing — not just “time off” — to guide return to sport. This includes:
Strength testing (quad, hamstring, adductor symmetry)
Hop and landing control tests Change-of-direction drills under pressure
Sport-specific reactivity and fatigue testing
Only once you pass these benchmarks — and feel confident — will we clear you to return.
At the end of your initial treatment, your action plan will be discussed and a expected recovery time will be established. If you have not recovered in that time, lived up to the expectations and there have been no unforeseen events, we will continue to rehabilitate your injury without additional charge!
