What Is a Syndesmosis Injury?

Syndesmosis injuries, often referred to as high ankle sprains, involve tearing or stretching of the ligaments connecting the tibia and fibula. These injuries typically occur during twisting movements, heavy contact, or awkward landings — common in footy, netball, basketball, and soccer.

Signs you may have a syndesmosis injury:

Surgical vs Non-Surgical Treatment Options

The management of syndesmosis injuries depends on the degree of ligament damage and joint instability:


Low to Moderate Grade (Conservative Management)

Stable injuries can often be managed with bracing and a progressive rehab plan — focusing on restoring range, loading capacity, and strength.


High Grade or Unstable (Surgical Management)

More severe injuries, or those with significant separation of the tibia and fibula, may require:

More serious or surgical cases may take 12–16+ weeks, depending on progress and sport demands.
